Using your out-of-network benefits

Madison Medical Care doesn’t accept insurance, so you pay for each visit yourself. If your plan has out-of-network benefits, your insurance company may pay back part of the cost.

How it works

  1. 1

    Pay for your visit

    All visits are self-pay.

  2. 2

    Ask us for a superbill

A superbill is an itemized receipt with the details your insurance company needs.

  3. 3

Send it to your insurance company

Most insurers let you submit it online or by mail.

  4. 4

Your insurance company decides what to pay back

How much you get back depends on your plan.

Questions to ask your insurance company

  • Does my plan include out-of-network benefits for specialist visits?
  • Have I met my out-of-network deductible this year?
  • What share of the cost does my plan pay back?
  • How do I submit a claim, and is there a deadline?

Plans that may include out-of-network benefits

If you have a PPO or POS plan from a company such as Aetna, Cigna, UnitedHealthcare, Oxford, or Empire Blue Cross Blue Shield, it may include out-of-network benefits. HMO and EPO plans usually don’t. Madison Medical Care isn’t in network with any insurance plan, so check your own plan’s details.

What your insurance still covers

Lab tests, imaging, and prescriptions go through your insurance as usual.

Who we can’t see

We can’t see patients enrolled in Medicare (including Medicare Advantage) or Medicaid.

We can’t promise your insurance company will pay you back. Reimbursement depends on your plan.
